1. About this policy
The controller of personal data submitted through this website is sole proprietor Yurii Mykolaiovych Dmytrychenko (ФОП Дмитриченко Юрій Миколайович). This policy covers dentlayt.com and requests submitted through its booking form. It does not replace the information about medical records or informed consent to treatment provided at the clinic.
2. Information in your request
The form sends your name, phone number, preferred contact method, selected clinic address, optional comment and a technical request identifier. You do not need to add a comment.
Contact details are sufficient to request an appointment. Please do not include diagnoses, examination results, passport details or payment information in your comment. Discuss medical matters directly with your clinician.
3. Why your information is needed
Request details are used to contact you, clarify your enquiry and arrange a visit to the selected clinic. Before submitting the form, you confirm consent to processing your information for this enquiry. You can call the clinic instead of using the form.
Booking consent is not consent to advertising messages. The grounds for processing information during subsequent treatment are addressed separately.
5. Analytics and browser storage
The website integrates Vercel Web Analytics for visit statistics, including pages viewed, referral source, device type, browser and approximate location. According to Vercel documentation, this service does not use third-party tracking cookies.
The website stores an md-loader-done flag in sessionStorage to avoid repeating the loading screen during a tab session. It does not contain your name or phone number. The current version has no Google Analytics, Google Ads or advertising-cookie settings banner.
6. Storage and access
To ask how long your request is retained or request deletion of its data, contact the clinic administration using the details below.
Medical record retention requirements differ from those for website enquiries. Deleting an enquiry does not automatically delete records that the clinic is required by law to retain.
7. Your rights and requests
You may request access, correction or deletion of your data, object to processing or withdraw consent where provided by applicable law. Withdrawal does not affect the lawfulness of prior processing. We may need to verify that the request concerns your own data.
Tell the clinic administration that your enquiry concerns personal data and provide the phone number and approximate date of the original request. You may also seek help from the Ukrainian Parliament Commissioner for Human Rights or a court.
